HHA Required Documents:

• NYS Certified Home Health Aide Certificate
• I-9 Documentation (Ex: Unexpired NYS Photo ID)
• Social Security Card
• Two (2) Reference Letters
• Physical from within six (6) months including lab work

1.     Physical must include PPD. If the PPD is positive, then a chest X-Ray is required

• Rubella and Rubeolla Titers
• Proof of Current Flu Vaccine
• Drug Screen within six (6) months

Applicants bilingual in Russian,  English, Spanish, Creole, Greek, Italian, Urdu or Japanese is a huge plus!
