Amity Care Home Health provides skilled nursing and rehabilitative care in the comfort of your own home. Private-duty services also are available. Passport Home Care Services provides therapy and social services to allow a person to remain in their home as long as possible. Amity Care operates two central Ohio offices with home health services available 24 hours a day, 365 days a year.

Amity Care Circleville serves Fairfield, Fayette, Hocking, Pickaway, Licking and Ross counties

 Scheduled Hours per week: 

Part-Time 30 hours or less; ability to work all shifts along with evenings and weekends.

 Position Summary / Role and Purpose: 

The purpose of this job is to ensure clients receive appropriate personal care, homemaking tasks and are enabled to attain their optimum level of functioning according to client’s care plan under the supervision of a registered nurse.

 Minimum Qualifications: 

• Education. High School Diploma or GED.

• Certificates, Licenses, Registrations.

• Possess a Nursing Assistant Certification from the State of Ohio that is in good standing.

• Possess a valid driver’s license from the State of Ohio.

• Possess proof of current car insurance.

• Experience. 1-2 years experience in home health care and in the management of client care.

• Technical experience with Patient Care/Management systems 

 

 Preferred Qualifications: 

• Possess a Nursing Assistant Certification from the State of Ohio that is in good standing.

• Possess a valid driver’s license from the State of Ohio.

• Possess proof of current car insurance.

• Experience. 1-2 years experience in home health care and in the management of client care.

• Technical experience with Patient Care/Management systems.

  Additional Job Information: 

• Accountable for client’s care in conjunction with Clinical Services Manager and other disciplines within Agency to ensure quality care.

• Observe client within patient care policy guidelines and immediately report any conditions requiring attention to supervisor.

• Maintain clients' home by changing bed linens and removing trash. Maintain appropriate grooming supplies are available in client’s home.

• Ambulate and transfer client as needed utilizing appropriate assistive devices and body mechanics.

• Record client’s vital signs, weights, measurements, intake and output, food consumption and other information as assigned

• Perform range of motion and reality orientation with clients assigned and developed by client’s care plan.

• Accountable for compliance, excellence, and delivery of client care in adherence with organization, local, state and federal regulations.

• Records documentation and reporting related to client’s care to ensure regulatory and organizational requirements in a timely and accurate manner.

• Records and reports time and mileage accurately and provides to management. Reports any conditions requiring attention to supervisor.

• Attends a minimum of one safety meeting annually as well as two mandatory in-services.

• Assures that the policies and procedures around confidentiality/rights of all clients and their care information are maintained.

• Conduct daily client rounds and report problems to Clinical Services Manager Communicates and works with Client’s family and/or responsible party to develop positive relationship and ensure necessary information sharing.

• Assures that the client’s personal and private property rights are followed by staff at all times.

• Interacts with clients to ensure that quality care and positive client/staff relations are taking place.

• Maintains open and trust-building relationships with client family members, responding professionally and promptly to any requests or concerns
