About the Opportunity
The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care. Additionally, this individual will lead the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM). These duties are to be performed in a timely manner consistent with Centers for Medicare and Medicaid (CMS) guidelines and Trans Health standards. This individual also establishes and maintains positive relationships with all members of the healthcare team.

Essential Functions (summary):
Maintains current MDS status of each patient/resident, according to state and federal guidelines as required under the Omnibus Budget Recomciliation Act (OBRA), Medicare PPS and any Medicaid payment system, as applicable.
Schedules patients/residents for appropriate OBRA, Medicare PPS and or Medicaid required assessment.
Implements system to monitor for changes in patient/resident condition.


REQUIREMENTS:

Qualifications
•Ideal candidate must be a skilled communicator, director and motivator. Able to organize and prioritize many tasks effectively.
•RN with unencumbered license to practice as an RN.
•Able to react to emergency situations appropriately when required.
•Must be able to read, write, speak and understand the English language
•Must be able to relate information concerning a patient/resident's condition.
