Social Worker - Acute - (SW - Acute)

Social Worker Team Lead needed for a 3 month assignment to start in Sept/Oct.

Social workers are responsible for providing psychological support to people, families or vulnerable communities to give them the coping mechanisms they need to combat ailments. Social workers also provide advice for family caregivers, counsel patients, and help organize the patient’s treatment plan following discharge from the facility. The social worker works in a variety of settings, such as:


  • Acute
  • Home health
  • Long-term care
  • Outpatient


Required Qualifications
Master's Degree in Social Work from an accredited graduate school of social work.

Two or more years of clinical experience with designated population; hospital, emergency room, chemical dependency, or community agency experience preferred.

Knowledge of community resources, knowledge of Medicare, Medicaid and insurance requirements as these relate to continuing care
Licensed Clinical Independent Social Worker (LICSW).
CCM or working towards, preferred.

Preferred Qualifications
Previous leadership experience preferred.
